Notice of Privacy Practices

Our commitment

Apotheosis Medical Aesthetics is required by law to maintain the privacy of your protected health information (“PHI”), to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. Washington law provides protections that are in some cases stronger than federal law; where state law is more protective, we follow state law.

How we may use and disclose your health information

Treatment. We use your health information to provide your care — for example, sharing relevant history between your Apotheosis providers, or with another provider involved in your care.

Payment. We may use your information to bill and collect payment for services you receive.

Health care operations. We may use your information to run our practice — quality review, training, scheduling, and appointment reminders by phone, text, or email. Tell us if you prefer to be reached a particular way.

As required or permitted by law. We may disclose information when the law requires it — for example, for public health reporting, to prevent a serious threat to health or safety, or in response to a valid legal order.

Any use of your information not described in this notice — including marketing, photography and treatment imagery, testimonials, and any sale of information (which we do not do) — happens only with your separate written authorization, which you may revoke at any time.

Photography and imagery

Clinical photographs are part of your record and are never shared publicly — on our website, social media, or anywhere else — without your explicit written consent, which is specific, optional, and revocable.

Washington state protections

Under Washington’s Uniform Health Care Information Act, most disclosures of your health care information require your specific written authorization, and certain categories — such as reproductive health, mental health, and sexually transmitted infection information — receive heightened protection.

Washington’s My Health My Data Act further protects health information collected outside the traditional patient record — see our Privacy Policy for those rights, including access, withdrawal of consent, and deletion.

Your rights

You have the right to:

• Inspect and receive a copy of your health record, in the form you prefer, within the timeframes Washington law requires.
• Request corrections or amendments to your record.
• Receive an accounting of certain disclosures we have made.
• Request restrictions on how we use or share your information, and confidential communications at a specific phone number or address.
• Receive a paper copy of this notice on request, even if you agreed to receive it electronically.
• Be notified promptly if a breach ever compromises the privacy or security of your information.
• Choose someone to act for you — a personal representative with medical power of attorney can exercise these rights on your behalf.

Changes to this notice

We may update this notice, and the new version will apply to all information we hold. The current notice is always posted here and available at our front desk.

If you have questions about this notice or believe your privacy rights have been violated, contact our Privacy Officer using the information below. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
